Entry Forms

Please email/phone/fax to obtain information on an Entry Form for Champions Dance 2010. The entry form will be emailed to you (Please indicate Microsoft Office Excel or Microsoft Works Worksheet). Simply complete the required information and email it right back. Established computer programs, with required information are acceptable to send.

ENTRY FORMS MUST BE RECEIVED BY JANUARY 15, 2010.
ENTRY FEES MUST BE RECEIVED 6 WEEKS PRIOR TO FESTIVAL.

In order to honor our time restrictions, Champions Dance will not exceed 215 entries per 13½ hour day. All entries are received on a first come, first served basis and will be limited to the amount of performance time available. Send your entries early, if you wish to secure your placement.

ENTRY FEES ARE NON-REFUNDABLE.
